*An Evaluation Consent form will be required for scholarship acpplications to be reviewed. Evaluation Consent forms should accompany the scholarship application. Student Scholarship Applications (located under Family Advocacy) are due April 11, by NOON, to Thomas Ray at 4001-E Carya Drive, Raleigh, NC 27610.
